I've bought from Adaptec before. Mike is great to deal with and will ensure all your specs are catered for. He has experience with most wheel fitments and could possibly offer you extra assistance you might not be aware of. I bought 5x100 --> 5x112 adapters from him and they were cut from T6061 billet aluminium (light-weight). He supplied the bolts required to secure the adapter to the hub. At first I wanted pressed studs so I could use wheel nuts to secure the wheels but he suggested to have the adapters tapped instead because there's a chance I might use Mercedes wheels (he said they usually have thicker wheel hubs and so the studs may not reach out all the way). I didn't end up using Merc wheels but the adapters worked well - threads were cleanly tapped, screwed on perfectly without much play with my stock VR6 wheel bolts. 15mm is the thinnest adapter he can make and they're fastened with flat-seat bolts. I also got 25mm adapters from him and they were fastened using conical-seat bolts. Again, cleanly cut. Bolted up to the hub fine. I torqued them down to wheel-spec (I use 90 ft lb) along with some blue Locktite and it got me down to Melbourne and back. I would definitely buy from him again. When dealing with Mike, I got the feeling that if I was a complete noob at wheels and such, I would still be able to buy what I want from him as he's easy to talk to and would guide you every step of the way. I got frequent updates and the shipping was quick. Ended up costing me ~$450AUD when the dollar was awesome.

I've also bought from ADAPTITUSA, 5x100 --> 5x114.3 in 25mm and 33mm sizes, also made from T6061 billet aluminium. Service was less personal but was still very straight forward and easy if you know the specs that you want. Thinnest they'll do though is 20mm. The adapters came with pressed studs - to secure the adapter to the hub, they provided threaded studs and nuts (like as if to convert our hubs to use wheel nuts). They didn't provide the nuts to secure the wheel to the adapter though, and didn't provide me with info on what size wheel nuts to use. They only offer shipping within the US (free), so I had them ship the adapters to a mate of mine and had him send the adapters over to me. Only ended up costing me ~$400AUD when the dollar was good.
I'd highly recommended Adaptec.
