Yes you replace both, but i only got photos of one of them. One bushing is a circular shape, and the other is rectangular. The one in the pics is the circular one.
The stock ones are a hollow rubber.

Word of warning, you may want to watch the location of the shift cables after you fit the dump of choice, some are simply too close due to their ill thought-out design and the gap is even less after the fitment of a SS kit. The shift cables are pretty badly held in position where they exit/enter the firewall and they're easily moved if your not careful. One totally stock R with DSG in Sydney has had an engine bay fire due to this.
This is something we always look at now and if we can't move it we make the owner aware of the potential issue so they can either fab up some sort of bracket or at the very least fit some sort of heat-shielding. Even if it doesn't catch on fire the inner core low friction material around the cable core can melt. I aint an issue on the TXS pipes of course because we ran into this issue ages ago on one of the trial cars and re-did the whole top section before their release.

By SS do you mean short shifter? I don't see how a kit could lessen the gap you're talking about. The example you gave was for a DSG, so I'm a bit confused by your post.

Both the DSG and 6mt are manual boxes at heart and with the 6mt the short shift cable tends to push it the wrong way - toward the dump . You'll see what I mean
Yep my 42DD downpipe rubs on the cables. Im keeping a close eye on them incase of damage due to the heat/pressure etc. My old Scorpion downpipe had about 2cm clearance but the 42DD downpipe doesn't bend as tightly after it exits the turbo and therefore hits the shifter cables.
No issues at this point in time but im keeing a close eye on it.
